Description:
The Judge Group is seeking PRN Nurse Practitioners to conduct in-home health assessments for Medicaid/Medicare Advantage Members with a leading healthcare provider in Bannock County, ID.

Job Summary:

In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare and other populations. Your primary objective will be to assess the overall health and well-being of member beneficiaries to ensure accurate and comprehensive risk adjustment coding, leading to greater value-based care.

Schedule:

• M-F during business hours is preferred but weekends are okay.

Minimum Assessments:

• A minimum of 30 assessments must be completed each month.

Responsibilities:

• Perform annual wellness visits and health assessments on a population with chronic conditions

• Deliver patient health education opportunities

• Assist in closing quality care gaps (i.e. screenings and labs)

• An enthusiastic collaborator contributing to the enhancement of care delivery

• Expected to cover a maximum 55-mile radius

• Providers are expected to commit a minimum of 30 hours per month

Qualifications:

• Must have a valid unencumbered NP License for the state you will be working in

• Previous in-home Risk Assessment experience preferred

• Will take any background/specialty besides pediatrics

• Will consider new grads
