General Summary/Overview
This position will work Southwest of Boston.
This Physical Therapist position will be part of a clinical team that provides acute-level care to patients in the comfort of their own home. The MGB Home Hospital (HH) is an innovative strategic program that aims to improve the care of patients while increasing hospital capacity and reducing total costs of care by providing physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease, or other causes. Patients admitted to Healthcare at Home, are receiving acute care and post-acute care in their homes. Patients may range in age from 18 +. Collaborate with patient, family, physician, advanced practice provider, nursing and other allied health professionals to identify patient needs and coordinate services to meet those needs.

Key Responsibilities
• Develop a plan of care that is patient and family focused and institutes standardized interventions for goal-oriented outcomes.
• Physical therapist may conduct virtual and or in person patient evaluations, depending on patient needs, and established Healthcare at Home criteria.
• Daily documentation is accurate and timely to promote real time updates visible to full clinical care team
• Application of standard treatment procedures
• Applicable teaching is provided to patients and caregivers.
• Age-appropriate evaluation is completed that reflects patient history, relevant systems review and appropriate tests and measures and reported to primary clinician.
• Plan of treatment is reassessed daily while admitted to HH service, plan is modified and communicated as appropriate to meet discharge goals.
• Provider orders are current and consistent with plan of care and documented in the medical record.
• Documentation is complete and submitted timely according to agency standards.
• POC devices are updated as directed.
• Verification of Payor information on admission is verified in computer system and attended according to agency standards.
• Documentation to support Managed Care authorizations/DME requirements is maintained/transmitted within designated time frames.
• Visit schedule is consistent with authorizations by Managed Care payors.
• Reportable outcomes meet patient goals.
• Realistic, objective, time-oriented goals for patient performance are identified based on examination findings.
• Treatment and discharge plans are selected based on examination findings and patient goals.
• Patient is informed of his/her weekly schedule and any other changes to their schedule as necessary.
• Receives positive comments from patients/families/referral sources/co-workers/management.
• This dynamic role has cross functionality with the HH Navigator. This person will serve as Navigator providing patient identification and collaborate with the admitting triad on HH admissions.
Qualifications
Licensure, Certifications or Registration
Registered with the Board of Registration in Massachusetts.
Current valid driver's license, a good driving record, and reliable transportation required
Experience
• Minimum of two (2) years' experience as a licensed Physical Therapist; homecare and/or acute care experience preferred
• Registered with the Board of Registration in Massachusetts.
• Strong customer service skills and ability to work with diverse populations.
• Excellent verbal, written and electronic communications skills
• Computer proficiency and ability to learn new software; Epic and Rover experience preferred
• Ability to prioritize work and adapt to changing situations.
• Travel required within MGB Healthcare at Home geographic area.
• Must possess spirt of innovation and progressive thinking, and creativity.
Physical Environment
Primarily will work in patients' homes requiring driving between sites. Potential exposure to blood, blood products, body fluids and chemical hazards. Attend meetings in a variety of external organizations, healthcare facilities, hotels and other venues.
Location
Will be hired into a designated geography; may be asked to travel beyond designated region of preference to support operational needs
