About the SUD Bridge Clinic

  • The Substance Use Disorder (SUD) Bridge clinic will provide on-site rapid assessment, treatment initiation, and stabilization for patients with substance use disorder, with a specific focus on equitable care which seeks to eliminate any disparities in access to or retention in treatment by race, ethnicity, or language. This Bridge clinic will be staffed by a multidisciplinary team, which includes a physician and may also include a therapist, a recovery coach, an advanced practice provider, social worker, and/or an office-based addiction treatment (OBAT) nurse care manager.
  • The care model will consist of rapid access to low threshold evaluation and treatment for individuals identified with SUD throughout the Mass General Brigham system.
  • In this model, the patient is at the center of the team, and the nurse works collaboratively to manage and coordinate care, triage urgent clinical needs, provide patient education, administer medications, offer telephonic support, participate in prescription management and other administrative duties.
  • Pharmacotherapy will be initiated promptly for appropriate patients and patients will be connected to other Bridge clinic services, including therapy, peer support, harm reduction services, and nursing support.
  • The patient will be managed by the Bridge Clinic team on a transitional basis while linkages are made to the suitable and clinically appropriate level of care within the community. Patients who cannot directly be connected to ongoing addiction treatment services will continue receiving care at Bridge Clinic until linkage is successfully achieved.
About this Staff Nurse/ Care Manager Job:

Principal Duties and Responsibilities:
  • The office-based addiction treatment (OBAT) nurse care manager in conjunction with the Bridge Clinic team, helps manage and treat patients with SUD in a welcoming, low-threshold, and person-centered and trauma-informed care model.
  • The OBAT nurse serves as a care manager partnering with patients to improve treatment initiation, retention, and long-term clinical outcomes and may work as part of the SUD Bridge clinic team and/or primary care settings to support the transition of patients from Bridge Clinic back to primary care.
  • The OBAT nurse follows MGB, state and federal guidelines to support individuals with SUD in accessing, navigating, and being treated effectively in the Bridge Clinic and primary care. The OBAT nurse works with the Bridge Clinic team and primary care team to establish and implement protocols for medical management, substance use disorder treatment and pharmacotherapy, stabilization in Bridge Clinic, connection to additional needed SUD treatment or recovery services and linkage to treatment within the primary care practice.
Additional duties and responsibilities:
  • Assesses, plans, implements, documents, coordinates, monitors, and evaluates the plan of care by collaborating with the patient and the Bridge clinic providers and/or the primary care team.
  • Supports patients in initiating and maintaining on pharmacotherapy as clinically appropriate and decided upon by patient and provider.
  • Administers injections of extended-release medications and provides training and technical assistance to primary care practices to implement extended-release medications in the primary care practice.
  • Provides harm reduction education and interventions to patients, including safer use training, overdose education, naloxone training, and drug checking education. Develops and maintains patient education materials in coordination with the department's clinical team.
  • Researches, evaluates, and recommends resources to meet the medical and non-medical needs of patients. Works with the Bridge Clinic team to initiate referrals to the appropriate agency or resource and conducts regular reassessment of patient needs and resources.
  • Functions as a liaison to promote effective working relationships between Bridge Clinic team and primary care staff and other agencies and resources.
  • Consistently provides high quality and timely documentation, including admission notes, progress notes, telephone encounters, discharge summaries and dictations.
  • Acts as a resource and consultant for primary care teams and serves as a nurse champion for SUD treatment.
  • Maintains current clinical knowledge base of SUD treatment and harm reduction.
  • Proactively performs needs assessment and collaborates with the treatment team to develop the plan of care.
  • Collaborates and communicates with the Bridge Clinic team, primary care staff, and other involved staff/agencies to ensure appropriate coordination of services, including coordinating supported referrals. Tracks services to monitor adherence to treatment plan, and to avoid duplication of efforts.
  • Assists the attending physicians and other prescribers in any potential collection and analysis of validated quality and outcome measure data. Collaborates on projects that impact overall departmental goals and initiatives.
  • Participates in departmental education and training to medical assistants, other staff members and students regarding the needs of the department.
  • Prepares, performs, and documents the coordination of clinical care, including assisting the patient with insurance and clinical components of paperwork, assisting with prescription refills, and other services to support the prescribers and other team members.
  • Collaborates with other team members on prior authorizations for prescriptions.
  • Monitor and obtain patient's toxicology screen and other lab tests as dictated by the OBAT program's guidelines. Educates patients and families on SUD management, effectiveness of medication treatment, harm reduction, and community resources and capitalizes on opportunities for education and referral.
  • Complies with confidentiality policies, HIPAA, The Joint Commission, and other regulatory requirements.
  • Completes BSAS trainings offered by MDPH and attends MDPH meetings as required.
  • Other duties as assigned by the supervisor/manager or leadership team.


Qualifications
QUALIFICATIONS
  • Graduate of an approved school of nursing with current registration in nursing in Massachusetts. BSN is required for newly licensed nurses.
  • Certified Addictions Registered Nurse (CARN) or willingness to obtain certification within first year of hiring required.
  • Experience with telephone triage and telephone-based clinical assessment techniques required.
  • Minimum 5 years clinical experience with experience in community health and substance use disorder services preferred.
  • Care management experience preferred.

  • Skills/Abilities/Competencies Required:
  • Experience working with a multidisciplinary team preferred.
  • Competent in basic principles of SUD assessment and treatment and understanding of pharmacotherapy for SUD, including buprenorphine and naltrexone.
  • Comfort and proficiency with harm reduction as a philosophy and set of interventions.
  • Skilled at providing person-centered care and general understanding of trauma-informed care principles.
  • Well-developed assessment/problem solving skills.
  • Excellent communication, organizational and interpersonal skills.
  • Ability to work both independently and as a team member in a multicultural setting.
  • Ability to provide appropriate care to patients of all age groups.
  • Remains proficient with BWH computer systems as needed to independently perform daily tasks such as order entry, EMAR, lab tests and results, incident reporting, etc.

    • May require car/driver's license.

    WORKING CONDITIONS:
    • Describe the conditions in which the work is performed.
    • Ability to sit for extended periods of time using a computer and telephone.
    • Ability to get in and out of vehicles during travel to agencies and main campus.
    • The work may require the lifting of heavy objects and patients, as well as exposure to potentially. infective material. Work requires contact with needles and other sharp instruments, monitoring devices, and other state-of-the-art equipment. Prolonged standing may be required. The hospital is approximately ¼ mile long and excessive walking to and from the patient care units can be expected.
