Position Description
This position is responsible for providing nutritional assessment, counseling, and education to patients and their families and for maintenance of pertinent information on nutrition for patient, medical staff, resident and student use. Work is performed primarily in the clinic setting, and employees function as part of the health care team. Work may require patient home visits.

Major/Essential Functions
Creates and completes nutritional assessments for patients, families, and providers in our Texas Child Mental Health Care Consortium (TCMHCC) programs.- May screen patients for nutritional risk(s).
- Provides nutrition counseling to patients and families according to specialty and scope of practice.
- Creates personalized nutrition plans for patients, families and providers that take into account the patients specific needs, preferences, and restrictions.
- Creates, updates and disseminates nutrition educational materials for use by patients, families, providers, clinical staff, education staff, and students.
- Coordinates educational activities, including scheduling, logistics, and follow-up communication with internal and external participants and venues. Educational activities may be performed using virtual, in-person, or hybrid methods.
- Establishes and maintains relationships and communication with interdisciplinary health care team.
- May evaluate the utilization and success of nutrition plans and education using various methods of assessment and feedback.
- Drafts, edits and prepares correspondence, reports and other material using word processing, spreadsheets and/or databases.
- Prepares and maintains program records.
- Maintains patient documentation in a thorough and timely manner within various electronic health record platforms.
- Prepares and organizes training materials and curriculum as necessary.
- Provides for smooth day-to-day flow of communications within the programs.
- Develops and updates monthly productivity reports.
