The Opportunity

Our Extended Care team consists of board-certified physicians, nurse practitioners, and physician assistants who provide episodic and chronic care to patients staying in several skilled nursing or long-term care facilities across the North Shore. Our dedicated care team closely monitors each patient's progress, coordinates and consults with specialists, and keeps both the patient's primary care physician and family informed. When it is time for a patient to return home, the team coordinates a plan of care with the patient's primary care physician, ensuring there is a smooth transition back to daily life.

As a potential job component for interested candidates, the Extended Care team also works in conjunction with our Home Visit Program. This offering is for patients who are temporarily or permanently unable to come into an NSPG office for appointments due to a long-term illness or chronic condition. The NP/PA works closely with a patient's physician to develop a care plan for long-term care, chronic disease management and/or an episodic condition.

The NP/PA is responsible for:

  • Assisting individuals and families with their care needs by obtaining health and medical histories, performing physical examinations and assessing health problems
  • Caring for patients with acute and chronic life-limiting illnesses by managing therapeutic regimens, prescribing medications and performing other studies and activities that are deemed appropriate
  • Managing and evaluating new and follow-up patients with physician input obtained as necessary
  • Assuming all SNF/LTC care management responsibilities
  • Facilitating communication with the patient, the patient's family or significant other, and the referring physician
  • Working in collaboration with the patients' attending physicians at the SNF/LTC facility, as well as providing continued collaboration with the patients' primary care provider(s)

The Schedule

This is a full-time, 40-hour position. Our NPs/PAs are required to see 10-11 patients (6-7 if also doing optional home visits) per day and must be available to respond to their pagers within 15-minutes between the hours of 8:00am and 5:00pm, Monday-Friday. The NPs/PAs are also required to be on-call and available via phone one night per week (5:00pm to 8:00am) and two weekend days per month (8:00am to 8:00pm). We do our best to publish the on-call schedule in advance for our NPs/PAs to plan accordingly and/or swap shifts if necessary. On-call is compensated.

The Locations

Potential skilled nursing or long-term care facility assignments include Beverly, Peabody, Lynn, and Salem. The optional Home Visit Program covers NSPG patients from Saugus to Rowley.

Qualifications
Qualifications

Education:
  • Master's degree in nursing or physician assistant studies required
License/Certification:
  • Certified Massachusetts Nurse Practitioner or Physician Assistant licensure required upon hire
Experience:
  • Prior SNF/home care experience preferred
