Lead Case ManagerCenter for Sport

Location: New Orleans, LA

Summary

Tulane University Center for Sport's Lead Case Manager will work within a comprehensive interdisciplinary medical team to assess patient care needs. The position will provide supervision of case management staff for the Center for Sport clinical programs. Supervises staff, including Social Workers and administrative support staff ensuring that cases are distributed appropriately, and case management services are delivered in accordance with contractual agreements and regulatory standards. This position will also be responsible for coordination of referrals to resources within the community to address medical, mental health, behavioral health, and social needs of patients.
Responsibilities may include: clinical and coordination of services management, patient assessment, treatment planning and coordination, multidisciplinary staffing, case management and ongoing program monitoring and development.

Required Qualifications

* Master's Degree in Social Work, Counseling, or Rehabilitation Counseling from an accredited program* Licensed Master Social Worker or Licensed Professional Counselor or Certified Rehab Counselor in Louisiana (or the ability to obtain before start date)* Five years of experience with direct patient care, case management and psychosocial issues of adults

Preferred Qualifications

* Licensed Clinical Social Worker.* Experience working with collegiate or professional athletes, active duty military and/or veterans.* Experience in mental health rehabilitation and complex medical and neurological conditions.
