Job Title: Hospice RN Case Manager

Duties & Responsibilities: Hospice RN Case Manager sought for 13 week assignment, with the potential to extend. Schedule would be 5x8 days, Monday-Friday. No weekends are required, but there would be a need for back-up on call. The area of coverage is Bedford/nashua/Manchester area.

Minimum Requirements:

• NH or Compact RN license
• 1+ year hospice in home settings experience within the past 3 years
• HCHB experience
• BLS
