Description:
Judge Healthcare is seeking a LPN for an opening with one of our top clients in the Clermont, FL area ASAP!
Position Details:
Title: Licensed Practical Nurse
Location: Clermont, FL 34711
Type of engagement: Contract
Hours: M-F 8am-5pm
Specific Duties:
- Case Management
- Clinical Support to Medical Assistants
- Working with Providers on high risk patients
- Daily, Weekly, Monthly follow up of High Risk patients
- Coordinating all post discharge, ER visit plan of care
Requirements/Qualifications:
- Florida LPN license- up to date
- Case Management experience
